A DBQ documents clinical facts: diagnosis, history, testing, symptoms, and functional effect. It does not grant service connection or set a rating. Whether a condition is connected to service can be a separate medical question, and the legal decision belongs to VA. See what you are being asked for before completing one.

The absence of a public Medical Opinion questionnaire does not stop a qualified clinician from writing a reasoned medical opinion in another appropriate format. That opinion is often the evidence a causation or aggravation question actually turns on.
